Where are you located?
We are located at 220 East Sevier Street, Benton, AR. All visits are by appointment. Our hours are: Monday 9am–5pm · Tuesday by appointment · Wednesday by appointment · Thursday 9am–5pm · Friday 9am–1pm · Saturday by appointment · Sunday closed.
What insurance do you accept?
We currently accept Aetna, Cigna, UHC, and Municipal Health. We are credentialing with BCBS, Ambetter, and Tricare. Insurance is accepted at LabCorp for all lab orders.
What forms of payment are accepted?
We accept cash, HSA, FSA, debit card, credit card, Cherry, and CareCredit.
What is the cancellation policy?
We ask that you provide at least 24 hours notice if you need to cancel or reschedule. This allows us to offer the slot to another patient in need of care.
What if I have a medical emergency?
If you have a medical emergency, please call 911 or go to the nearest emergency room. For non-life-threatening concerns, call us at (501) 313-0833 and we will do our best to fit you in.
Is my information private?
Absolutely. All consultations and records are HIPAA-compliant. Your health information is never shared without your consent.
